    About Us

    Elliot Behavioral Health Services is staffed by a team of mental health professionals who tailor both evaluation and treatment services to each individual patient. Psychotherapy services are available independent of or in conjunction with other treatment modalities, to provide an integral treatment approach to most problems. Medication evaluation and management services are designed to treat each individual patient's potential need and ability to benefit from medication intervention in biologically based conditions that provide an effective, yet less intrusive treatment approach. Communication of results and consultation with family, parents, schools and medical or mental health professionals, ensures that specific treatment approaches are implemented to meet each patient's need.

    Position Summary – M, W, F – Office, T, Th - Remote

    Behavioral Health Case Manager provides clinical social work interventions with patients, (pediatrics, adolescents, adults, and geriatrics) and families that that include assessing treatment needs, developing, monitoring, and evaluating treatment plans and progress.  Also provides services that empower clients to meet goals as independently as possible.

    Behavioral Health Case manager will address personal and environmental issues in order to maximize emotional, social and physical wellbeing and the appropriate and efficient use of health care services across the continuum. Promotes efficient coordination of patient care through collaborative practice with the health care team. Promotes patient, family, staff and physician satisfaction through coordination and continuity of care. Provides a systematic approach to evaluation of patient care. Coordinates patient care through a continuum of care model thereby facilitating the achievement of optimal quality outcomes in relation to clinical care and cost effectiveness.

    Key Responsibilities:

    • Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-focused
    • Provide on-going support and case management expertise to clients' needs
    • Educates and supports transition of patients to community-based services.
    • Ensure that all client case management plans are current and address identified needs;
    • Communicate effectively with a variety of individuals representing diverse cultures and
    • backgrounds and functions professionally in situations which require a high degree of sensitivity,
    • tact, and diplomacy.
    • Continually monitor and evaluate each client’s progression through their case management plan, and develop corrective action revisions to the plan as needed;
    • Maintain accurate documentation of service objectives and outcomes as well as other services in accordance with Federal, State, Solution Health guidelines;
    • Provides information, referrals, and facilitates and advocates linkages for clients to mental health treatment, temporary and permanent housing and public assistance programs;
    • Actively participate in client-related meetings and trainings;
    • Administer screenings for patients for mental health and substance abuse disorders.
    • Maintain client related data systems, including case notes and complete EPIC entries;
    • Prepare case management related reports including but not limited to outcomes, successes, etc.;
    • Adheres to strict boundaries and professional ethics in the care of others.
    • Ability to assess crisis situations and respond/refer appropriately in a timely basis.
    • BHCM role is primarily remote, and employee must utilize all designated communication tools to

    collaborate with EBHS team. Home office supplies will not be provided by Elliot Health System.

    Education/Experience/Licensure

    • Master’s degree in social work, Mental Health Counseling or related field
    • Experience in social service setting with working knowledge of case management systems and planning techniques

    M/W/F 8:00a-4:30p in office
    T/Th 8:00a-4:30p remote
