    Overview

    Patient Family Liaison Non-Clinical

    Location: St. Paul, MN

    Schedule: Monday-Friday with Flexible options and a weekend rotation/Hours: 9-6pm with 1 hour lunch

    Pay: $17.00 - $27.00/hr  (based on experience)

    Be the Best Patient Family Liaison You Can Be

    If you meet these qualifications, we would love to meet you:

    • Associates degree or higher preferred
    • Three to five years of professional sales experience preferred

    Responsibilities: 

    As a Patient Family Liaison, you will:

    • Collaborate with Admissions Department and Marketing staff to manage patient family information and sign-on visits.
    • Communicate appropriately with providers, attending physicians, other appropriate hospice staff members and other agencies as needed to ensure excellent provider relations.
    • Help the patient and family understand, accept, and follow medical recommendations. Provide services planned to assist the patient and family in achieving the optimum social, spiritual and heath care adjustments within their capacity.
    • Partner with staff on in-services, and education related to advanced care planning.
    • Maintain confidentiality of sensitive information per Health Insurance Portability & Accountability Act
