Position Title: (H2O) PTTS SPECIALIST

GENERAL SUMMARY
The Housing and Health Opportunities (H2O) Pre-Tenancy and Tenancy Sustaining (PTTS) Specialist is responsible for providing intensive, housing-focused supportive services to eligible members participating in the AHCCCS Housing and Health Opportunities (H2O) Program. Using a Housing First, trauma-informed, person-centered, and harm reduction approach, the PTTS Specialist assists members in obtaining, transitioning into, and maintaining safe, stable, and permanent housing while promoting independence, self-sufficiency, improved health outcomes, and long-term housing stability.
The PTTS Specialist provides medically necessary services that address housing-related barriers impacting the member's health and well-being. Responsibilities include completing Housing Needs Assessments, developing individualized Housing Care Plans, providing housing navigation, pre-tenancy support, tenancy sustaining services, independent living skills training, housing stabilization, landlord engagement, care coordination, transportation coordination, and community resource navigation. The PTTS Specialist collaborates with the Statewide Housing Administrator (HOM, Inc.), the AHCCCS H2O Program Administrator (Solari), healthcare providers, behavioral health providers, landlords, and community partners to reduce barriers to housing and support successful transitions to Long-Term Housing Programs (LTHPs).
The PTTS Specialist is responsible for monitoring member progress, updating Housing Care Plans as needs change, coordinating transitions between housing resources and supportive services, and promoting community integration through individualized interventions. The position maintains accurate documentation in accordance with AHCCCS H2O, Medicaid, HMIS, and agency requirements while exercising sound judgment, professionalism, and strong interpersonal skills when working with individuals experiencing homelessness, serious mental illness (SMI), substance use disorders, chronic medical conditions, justice involvement, and other complex health and social needs.
MAJOR DUTIES/RESPONSIBILITIES
Complete comprehensive Housing Needs Assessments and other required assessments to identify barriers to housing stability and independent living.
Develop, implement, monitor, and update individualized Housing Care Plans in collaboration with the member and interdisciplinary providers.
Provide Pre-Tenancy and Tenancy Sustaining Services that support members in obtaining, transitioning into, and maintaining permanent housing.
Assist members in developing independent living skills including budgeting, household management, medication management, personal hygiene, meal planning, transportation, and community integration.
Monitor member progress toward Housing Care Plan goals and modify interventions as necessary.
Standard: Housing Care Plans, assessments, and interventions are completed accurately and within required timeframes in accordance with AHCCCS H2O requirements.
Assist members with locating and applying for appropriate housing opportunities.
Coordinate housing applications, housing subsidies, reasonable accommodations, and supporting documentation.
Assist members in preparing for housing orientation, landlord meetings, lease signings, and move-in activities.
Provide ongoing tenancy support including lease education, landlord communication, conflict resolution, eviction prevention, and housing stabilization.
Assist members in developing financial plans to maintain long-term housing stability.
Standard: Members receive timely housing navigation and stabilization services that promote successful housing placement and retention.
Coordinate services with healthcare providers, behavioral health providers, Health Homes, the Statewide Housing Administrator (HOM, Inc.), the AHCCCS H2O Program Administrator (Solari), landlords, and community agencies.
Facilitate referrals to community resources including healthcare, behavioral health, employment, education, legal services, transportation, food assistance, and other supportive services.
Participate in interdisciplinary meetings, case consultations, and care coordination activities.
Advocate for members to reduce barriers to housing and improve access to community resources.
Standard: Care coordination activities are completed timely and support continuity of care and housing stability.
Maintain regular contact with assigned members in accordance with AHCCCS H2O requirements and SAMHSA Permanent Supportive Housing Fidelity standards.
Conduct home visits to monitor housing stability, safety, and member progress.
Provide crisis intervention and develop individualized housing stabilization strategies when housing is at risk.
Assist members in resolving barriers that may impact housing retention or community integration.
Facilitate warm handoffs and transitions to Long-Term Housing Programs (LTHPs) or other supportive service providers when appropriate.
Standard: Member contacts and interventions are completed consistently and support long-term housing stability.
Complete all required documentation, progress notes, Housing Care Plans, assessments, and HMIS entries within agency and AHCCCS required timeframes.
Maintain documentation that supports medical necessity, billing compliance, and program requirements.
Ensure compliance with AHCCCS H2O policies, Medicaid requirements, HIPAA, HMIS Data Standards, and agency policies and procedures.
Participate in documentation reviews, quality assurance activities, and corrective action processes as assigned.
Standard: Documentation is completed accurately, timely, and meets all contractual and audit requirements.
Participate in required supervision, staff meetings, case consultations, and interdisciplinary meetings.
Complete all mandatory agency, AHCCCS, and H2O required training.
Maintain competency in Housing First, Trauma-Informed Care, Harm Reduction, Motivational Interviewing, community resource navigation, and other evidence-based practices.
Maintain current certifications and credentials required for the position.
Standard: 100% compliance with required trainings, supervision, and competency requirements.
Maintain compliance with AHCCCS H2O, Medicaid, HMIS, HIPAA, and agency policies and procedures.
Deliver Pre-Tenancy and Tenancy Sustaining Services in accordance with the member's Housing Care Plan and program requirements.
Complete accurate, timely, and complete documentation that supports medical necessity, billing compliance, and program outcomes.
Coordinate services with healthcare providers, behavioral health providers, the Statewide Housing Administrator (HOM, Inc.), the AHCCCS H2O Program Administrator (Solari), and community partners as appropriate.
Participate in supervision, quality assurance activities, audits, and required training.
Ensure H2O services are not duplicated with other funding sources and report any compliance concerns in accordance with agency policy.
Standard: Maintains compliance with AHCCCS H2O, Medicaid, HMIS, and agency requirements as demonstrated through documentation reviews, quality assurance monitoring, audits, and supervisory evaluation.
Represent Achieve Human Services professionally in the community.
Participate in community outreach events, trainings, and collaborative initiatives.
Assist with program development, quality improvement initiatives, and special projects as assigned.
Perform other duties as assigned by the Housing Manager.
Standard: Assigned duties are completed accurately and within established timeframes.

Mental Application and Responsibility
This position requires the ability to prioritize and organize workload, handle numerous tasks simultaneously and work under pressure. Must be able to interact with people in person at client homes and community settings, on the telephone, must be able to interact with discretion and professionalism regarding confidential client information. Must be able to have sound judgement regarding behavioral health situations
This position involves high level responsibility for administering company policies and procedures and a moderate level of responsibility for problem resolution and making clinical and administrative decisions. Staff must exercise sound judgment and ethical standards in handling direct care and supervisory duties
Internal and Public Contacts
This position involves a high degree of interaction with staff, behavioral health and housing clients, family members, and other social service agency staff in the community on a regular basis
Conditions and Equipment
The work environment includes client home visits, clinical, and office settings for the provision of services and may include outside settings for outgoing and assisting customers with living skills, i.e., housekeeping, shopping, and personal hygiene. The employee must remain awake and alert throughout each work shift. Equipment operation includes general office equipment, computers, communication tools, and motor vehicles
Physical Demands
The work environment includes home and office setting. Staff must be able to walk up and down stairs and lift up to 25 pounds. The employee must complete trainings for safety and crisis intervention

Language (Communication)
Must be able to communicate clearly and effective orally and in writing; possess excellent organizational and time management skills. Bilingual English/Spanish verbal and written is a plus

Position requires a basic understanding of the Arizona State Housing Rules, Regulations and knowledge of Fair Housing and Landlord-Tenant Act Laws and understanding of HUD eligibility guidelines and Housing Contracts are required. QUALIFICATIONS
Education
Associate's degree or higher in related field OR
Three (3) to five (5) years of case management experience in lieu of education
Experience
Requires three (3) to five (5) years of case management

Must be over 21 years of age
Must possess a valid driver’s license
Must be able to obtain a Level 1 Fingerprint Clearance Card
Must be able to obtain valid certifications for Agency compliance within 90 days of employment
