We are seeking a compassionate and experienced Hospice RN Case Manager to provide patient-centered, end-of-life care in the Chicago area.

Schedule & Territory
- Monday–Friday, 8:30 AM – 5:00 PM
- On-Call One weekend rotation every 8 weeks
- Territory: Hyde Park, Evergreen Park, Chicago, IL 60805 and surrounding communities
Compensation & Benefits
- $80,000 – $97,000 annually (based on experience)

Key Responsibilities
- Manage a dedicated hospice caseload as the primary RN Case Manager
- Perform patient assessments, admissions, and individualized care planning
- Deliver skilled nursing care focused on comfort and dignity
- Coordinate care with physicians and interdisciplinary team members
- Conduct routine visits and adjust care plans as needed
- Provide end-of-life support, including family education and pronouncement
- Maintain timely, accurate documentation
- Supervise Home Health Aides in compliance with regulations
Qualifications
- Graduate of an accredited RN program
- Active Illinois RN license
- Current CPR and First Aid certification
- Hospice or home health experience preferred
- Strong clinical judgment and communication skills
- Valid driver’s license and proof of insurance
