Speech-Language Pathologist - PRN

AuthoraCare Collective is currently seeking a PRN Speech-Language Pathologist Case Manager for our Home Health department supporting Alamance/Rockingham Counties. Must be able to travel to, enter, and function in the homes of any patient within the service area.

The Speech Therapist provides speech or language therapy services in accordance with the patient’s plan of care as directed by their primary care provider.  Assess and develop home exercise plan individualized to patient. Complete comprehensive assessment to include cognition deficits, medication management, swallowing difficulties, speech deficits and provide education and rehabilitative/restorative treatment plan.

Manage the speech therapy Plan of Treatment for a case load of Integrated Health patients in collaboration with other members of the interdisciplinary team (IDT). Assess and identify patient problems, establish therapy goals, and interventions.  Work with physician to establish a treatment plan using a variety of methods, including exercises, hands-on therapy, and equipment to facilitate recovery.  Establish and maintain therapeutic relationship with patient, including providing community support for resources and education. 

Document the goals and outcomes of care. Therapist may perform evaluations and treatments for hospice patients as needed. Education and Experience:

• Master’s or doctoral degree in speech-language pathology.
• Minimum of one-year speech therapy experience home health or facility preferred.
• Current and unrestricted Speech Therapy Licensure.
• Current CPR certification.

Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
