Nurse Practitioner or Physician Assistant with an active North Carolina State License Location:

- Maximum of 40 hours per week (including breaks and administrative time)
- Expected average patient load of 2 visits per hour
- Collaborative environment with onsite lab, behavioral health therapists, and nutritionists
- Innovative, integrated use of telehealth
- Centralized care coordination, care management, and population health management
