Medical Provider - Family Nurse Practitioner (FNP) to provide comprehensive primary care services within our integrated outpatient behavioral health model. This position is based primarily at the Mofford Integrated Clinic and serves patients enrolled across Axiom Care programs through a mix of in-person and telehealth visits.
The FNP functions as a core member of an interdisciplinary care team, collaborating closely with behavioral health clinicians, nursing staff, care coordinators, and peer support specialists to deliver coordinated, whole-person care to a complex and underserved patient population.

Duties/Responsibilities
· Provide comprehensive primary care services, including assessment, diagnosis, treatment, and management of acute and chronic medical conditions
· Conduct patient visits in-person and via telehealth, as clinically appropriate
· Collaborate closely with behavioral health clinicians and other care team members to support integrated treatment planning
· Participate in daily team huddles, care conferences, and interdisciplinary meetings
· Prescribe medications and manage medication regimens in accordance with Arizona scope of practice laws, collaborative agreements (if applicable), and organizational protocols
· Order, review, and interpret diagnostic tests and laboratory results
· Maintain timely, accurate, and compliant medical documentation in the electronic health record
· Follow all applicable federal, state, and payer requirements, including AHCCCS/Medicaid regulations
· Support quality improvement initiatives and implementation of integrated care best practices
· Seek consultation from supervising or collaborating physicians as clinically indicated
