    Job description

    Overview

    The Home Health Aide will provide home health patient/resident care and performs medication administration and various treatments under the direction of the licensed nurse as required. The Home Health Aide completes assigned tasks to ensure timely, efficient and safe patient/resident care. Communication and interactions with patients, residents, families and other health team members is an essential component of this position.

    Responsibilities

    • Performs initial and ongoing data collection on home health patients/residents under the direction of the Registered Nurse/Licensed Practical Nurse.
    • Implements plan of care developed by the Registered Nurse based on patient/resident physical, psychosocial, cultural, ethnic and religious/spiritual needs.
    • Provides direct patient/resident care and reports condition or status changes accurately and timely to the RN or LPN.
    • Demonstrates time management and prioritization of tasks.
    • Documents results of patient/resident care within established policies, procedures and scope of practice.
    • Follows facility policies on medication administration and treatments under the direction of a licensed nurse.
    • Promote the Benedictine Mission and Core Values of Hospitality, Stewardship, Respect and Justice by bringing the Mission and Core Values into the day-to-day activities of the company.
    • Performs other duties, tasks and/or projects as assigned.

    Qualifications

    QUALIFICATIONS REQUIRED

    • Must be in good standing on the State Nursing Assistant registry or appropriate entity
    • Must complete required training for medication administration and various treatments
    • Valid Driver’s License
    • Acceptable driving record that meets Benedictine requirements.

    QUALIFICATIONS PREFERRED

    • Previous home health, hospital, or nursing home experience preferred
    • BLS Certification.

    Salary Range

    $20.50-22.00
