Day Program Case Manager

Mon-Fri  8-3:30  37.5 hrs/wk

Responsibilities:

• Under the guidance of the Program Director, design, develop and implement day habilitation service plans that facilitate individual progress
• Complete annual assessments, support strategies, day habilitation support plans, quarterly summaries and other documentation as required by day habilitation regulations
• Support and encourage individuals in meeting their habilitation goals
• Provide direct instruction to individuals.

• Bachelors Degree in related field required, plus two years of experience with individuals with disabilities or in other educational/human service setting
• Valid driver’s license and insured vehicle
