FT Resident Care Manager, LPN, Assisted Living (7am-3pm)

We are seeking a Licensed Practical Nurse (LPN) Resident Care Manager to provide leadership, clinical oversight, and team development within our Myers Hall Assisted Living community.

What You Can Expect:

As the LPN Clinical Supervisor, you will serve as a key leader for our nursing and direct care teams by:

• Provide daily leadership, guidance, and support to nursing and direct care staff.
• Foster a positive, accountable, and high-performing team culture.
• Assist with hiring, onboarding, staff evaluations, coaching, and corrective action.
• Lead staff education, training initiatives, and competency development.
• Conduct resident rounds and oversee quality clinical care delivery.
• Coordinate admissions, transfers, assessments, and care planning activities.
• Partner with physicians, hospice providers, home health agencies, and families to support resident needs.
• Ensure timely and accurate clinical documentation and physician order management.
• Oversee medication management processes and regulatory compliance.
• Facilitate care plan meetings and maintain person-centered care plans.
• Monitor quality assurance initiatives, clinical audits, and performance improvement activities.
• Respond to resident, family, and team concerns with professionalism and urgency.
