What You'll Do:
The ACT Nurse Case Manager is an integral member of the Assertive Community Treatment team and provides nursing, medication, physical health, care coordination, rehabilitation, and support services in community-based settings. The position supports individuals in advancing their self-identified goals related to recovery and wellness. Nursing services are delivered in collaboration with the psychiatric care provider, ACT team, primary care providers, pharmacies, hospitals, community partners, and each individual’s identified supports, when authorized. This role requires flexibility to transport clients/patients to organization-sponsored services or resources that address their basic and clinical needs. Drivers transporting clients/patients or driving company vehicles are to be age 21 or greater. For those using their personal vehicles to transport clients/patients, a certificate of insurance is required demonstrating at least the minimum required NC auto liability insurance.
