Physical Therapist (PT) - to work in Menagha and Park Rapids, MN Blue Stone Therapy is seeking (TEMPORARY position up to full time hours to cover a leave ) Physical Therapist (PT ) to provide rehabilitation services to adult and geriatric patients at at Green Pine Acres and Heritage Living Centers.

This position would ultimately settle into being PRN. This role focuses on evaluating and treating adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The Physical Therapist is responsible for patient assessments, care plan development, and implementation of treatment programs designed to improve gait, balance, mobility, strength, range of motion, pain management, and overall functional independence.

What You’ll Do

Perform physical therapy evaluations and develop individualized plans of care Provide skilled physical therapy treatment focused on functional and measurable outcomes Address gait training, balance, mobility, strength, range of motion, pain, and discharge planning Supervise Physical Therapy Assistants in accordance with licensure and regulatory standards Maintain appropriate patient schedules per established guidelines Complete timely, accurate documentation to support quality care and reimbursement requirements Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes
