The Director, Clinical Documentation provides strategic and operational leadership for the Clinical Documentation department, overseeing risk adjustment performance across CMS, Commercial, and Medicaid HCC models. This role drives accurate and compliant risk capture by leading department operations, monitoring RAF and key performance metrics, and executing strategic initiatives that improve clinical documentation and coding outcomes. The Director develops team leaders, partners with market, medical, and senior leadership, and translates performance data into actionable strategies. Success is demonstrated through strong RAF performance, consistent documentation and coding quality, regulatory compliance, and measurable improvement across markets.

Provide strategic and operational leadership for the Clinical Documentation department, ensuring accurate, complete, and compliant risk adjustment documentation and coding across CMS, Commercial, and Medicaid HCC models.

Drive RAF performance across markets, establishing performance goals, monitoring trends and opportunities, and implementing strategies to improve accurate risk capture, documentation quality, and overall program performance.

Lead, develop, and hold accountable the Clinical Documentation leadership team, including managers and trainers, while overseeing staffing, performance management, professional development, succession planning, and resource allocation.

Develop, prioritize, and execute strategic risk adjustment initiatives, translating organizational goals and performance opportunities into actionable plans with defined objectives, timelines, ownership, and measurable outcomes.

Deliver executive-level reporting and insights on RAF performance, coding and documentation metrics, provider performance, operational results, risks, and strategic initiatives to market, medical, and senior leadership teams.

Ensure regulatory compliance and coding integrity across all risk adjustment activities, maintaining adherence to applicable CMS, payer, ICD-10-CM, HCC model, audit, and organizational requirements while proactively identifying and mitigating compliance risk.

Partner cross-functionally with market, medical, analytics, operations, compliance, technology, and other key stakeholders to align risk adjustment priorities, strengthen provider engagement and education, resolve barriers, and advance organizational objectives.

Education and Skills

• Bachelor’s degree in Health Information Management, Healthcare Administration, Nursing, Business, or related field required; equivalent relevant experience may be considered.
• 7+ years of progressive experience in risk adjustment, clinical documentation, coding, healthcare operations, or a related field.
• 3+ years of leadership experience managing teams, leaders, or complex cross-market healthcare programs.
• Demonstrated experience with risk adjustment programs across Medicare/CMS, Commercial, and/or Medicaid populations.
• Experience managing RAF performance, coding and documentation quality, provider education, and operational performance metrics.
• Professional Coding Certification CPC, CRC CCS, or CCS-P required
• Advanced knowledge of risk adjustment methodologies, HCC models, RAF calculation concepts, ICD-10-CM coding, and clinical documentation requirements.
• Strong understanding of CMS risk adjustment requirements, coding guidelines, compliance standards, and audit considerations; working knowledge of Commercial and Medicaid risk adjustment models.
• Strong analytical skills with the ability to interpret RAF, coding, provider, and operational data and translate findings into actionable strategies.
• Excellent executive communication and presentation skills, including the ability to communicate complex risk adjustment concepts to clinical, operational, and senior leadership audiences.
