    Occupational Medicine Advanced Practice Provider

    Part-time
    Layton WorkMed - Intermountain Health

    Layton, UT

Based in Salt Lake City, Intermountain boasts a medical group, affiliate networks, homecare, telehealth, health insurance plans, and other services, along with wholly owned subsidiaries including SelectHealth and Intermountain Healthcare in Nevada.     • Competitive Compensation.
    • Hours/Shift: Part-time, 8AM - 5PM
    • Work locations: Layton WorkMed, Logan WorkMed, Ogden WorkMed, and Tremonton WorkMed

    Minimum Qualifications:

    • Current state license as a Physician Assistant or Advance Practice Registered Nurse (APRN).
    • Certification as a Physician Assistant or Nurse Practitioner (Credentialing requirement).
    • Eligible for State Controlled Substance License II through V.
    • Eligible for Federal DEA Certificate II through V.
    • BLS Certification for Healthcare Providers.
    • Other certification as required (e.g. ACLS, PALS, TNCC, NRP).

    Preferred Qualifications:

    • Master’s degree. Education must be obtained from an accredited institution. Degree will be verified.

    The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

    $57.33 - $88.47
