At Carefor, we know our work matters, and we’re proud of the impact we have in people’s lives.

You’ll bring a holistic approach to assessment, rehabilitation, and client-centered goal setting, while working closely with families, caregivers, and interdisciplinary teams to create meaningful outcomes.

    • Promote independence and prevention through early intervention, proactive safety strategies, and self-management support
    • Assist with funding applications for adaptive equipment, assistive devices, and home modifications
    • Collaborate with case managers, health professionals, and community partners to deliver seamless client care
    • Advocate for clients by connecting them to resources, services, and supports that enhance quality of life
