Job Details

Job Duties:

• Assess, plan, develop and provide the patient or responsible caregiver a home health plan of care for occupational therapy.
• identifies patient needs for other disciplines and services.
• Coordinate and evaluate the plan of care with all other disciplines, organizations and physicians for an assigned caseload.
• Communicates significant changes in patient status to the physician, rehab coordinator and the appropriate agency personnel when indicated.
• Demonstrates effective communication skills, keeping lines of communication open between Lead O.T. Supervisor, Team Managers and all staff involved in care of assigned patients.

Minimum Requirements:

• 2 years of experience as an occupational therapist
• Current registration to practice Occupational Therapy in New York State

Posted Salary Range

USD $90,000.00 - USD $115,000.00 /Yr.
