Now Hiring: Integrated Care Coordinator
Rock Island | Dental & Behavioral Health | $17.00–$20.00/hour
Do you love helping children and families get the care and support they need? We’re looking for a caring, organized, and community-minded person to join our team as an Integrated Care Coordinator!
In this role, you’ll help connect pediatric patients and their families with dental, behavioral health, medical, and community services. You’ll work alongside our care teams, meet with families, help remove barriers to care, and make sure patients don’t get lost in the shuffle.
What You’ll Do
- Connect children and families with the right care and resources.
- Complete dental and behavioral health screenings.
- Coordinate warm handoffs between CHC services.
- Help families schedule appointments and follow up on recommended care.
- Work with Community Health Workers to address barriers like transportation, insurance, and scheduling.
- Build relationships with schools, daycares, and community partners throughout Rock Island County.
- Participate in outreach events that help families learn about available dental and behavioral health services.
- Help create a positive, welcoming experience for every patient and family.
What We’re Looking For
- High school diploma or equivalent required.
- Experience working with children, families, healthcare, schools, or community organizations is a plus.
- Strong communication and relationship-building skills.
- Someone who is organized, compassionate, and great at following through.
- A team player who enjoys making a difference in the community.
A degree in social work, human services, psychology, healthcare, public health, education, or a related field is preferred, but not required.
Position Details
- Location: Rock Island
- Pay: $17.00–$20.00/hour
- Status: Full-time, non-exempt
- Schedule: Monday–Friday, between 8:00 a.m. and 5:00 p.m., with occasional evenings or weekends
- Travel: Local travel between CHC clinics and community locations may be required.
