The Clinical Pharmacist – Ambulatory Transplant is part of a specialized team of three decentralized transplant pharmacists supporting complex transplant and heart failure patients. This caregiver will primarily support the heart transplant and heart failure teams, collaborating directly with providers and participating in interdisciplinary rounds. The role also includes cross-training to provide coverage for liver and kidney transplant services, with opportunities to support inpatient clinical patients as needed.
A caregiver in this position will work Monday through Friday from 8:00am to 4:00pm. A caregiver in this role may need to float into inpatient. No weekend or holiday requirement.
