    Overview

    Seeking a Behavioral Health Consultant to provide counseling to patients and assistance to medical providers and other CBHA staff as part of Integrated Behavioral Health team. The ideal candidate will be instrumental in providing access to behavioral health to patients through brief solution-focused interventions, while generating internal and external referrals for specialty behavioral health needs.

    Behavioral Health Consultant salary range: $78,041.60 - $97,522.00 DOE with ability to go higher for highly experienced candidates.

    Responsibilities

  1. Works as part of the primary care team to manage patients with behavioral health problems efficiently and effectively.
  2. Care Management - provides timely interventions; transitions patients with chronic behavioral conditions to appropriate services; links families to resources and provide referrals; educates patients and staff regarding behavioral health care, prevention and treatment.
  3. Document interventions in the electronic medical record, in accordance with compliance requirements and guidelines.
  4. Participate in evaluation and training of peers and support staff.  Also, monitor behavioral health program, identify problems related to patient care and make recommendations for improvement.
  5. Other duties as assigned.
  6. Promotes a positive work culture and upholds clinic policies and procedures, including the WE CARE standards of Behavior.
  7. Must uphold confidentiality at all times in regards to CBHA activities, reports, financials, patient health information and other proprietary information specific to CBHA.
  8. Qualifications

    Professional and Technical Knowledge:

  9. Possesses an understanding in the application of behavioral health theory, case management interventions and ethical practices of the behavioral health profession.
  10. Requires the equivalent of extensive formal training in providing behavioral health and case management interventions, usually acquired in the completion of a Master’s Degree with a major in social work, counseling, or related field.
  11. Must be bilingual/biliterate in English/Spanish
  12. Licenses and Certification:

  13. Possesses a license to provide behavioral health interventions/counseling (LMHC, LMHC-A, LICSW.)
  14. Possesses a valid driver’s license.
  15. Technical Skills:

  16. Ability to prepare basic correspondence and simple reports in Microsoft Word.
  17. Ability to use Microsoft Excel to create tables and simple displays of information.
  18. Ability to create basic presentations in Microsoft PowerPoint.
  19. Ability to enter and manage data into a centralized electronic health records
