Position Overview
The Advanced Practice Provider is responsible for providing comprehensive, patient-centered outpatient medical care to individuals and families in a Family Medicine setting. Working within CHA's Patient-Centered Medical Home (PCMH) and Team-Based Care model, the APP evaluates, diagnoses, treats, and manages a broad range of acute and chronic health conditions across the lifespan, consistent with the provider's education, training, certification, and scope of practice.
The APP collaborates with an interdisciplinary team to promote prevention, wellness, access to care, and positive health outcomes. Depending on the health center location and operational needs, the position may also include providing continuity of care in inpatient and/or residential settings.
Key Responsibilities
- Provide comprehensive primary and family medicine care to patients of all ages, consistent with education, training, certification, licensure, and scope of practice.
- Evaluate patients, obtain medical histories, perform physical examinations, establish diagnoses, and develop appropriate treatment plans.
- Diagnose and treat a broad range of acute and chronic illnesses and injuries.
- Provide preventive care, health education, chronic disease management, and wellness services.
- Recognize and appropriately manage medical and surgical emergencies.
- Order, interpret, and follow up on appropriate diagnostic tests, laboratory studies, and other clinical services.
- Prescribe medications and treatments within applicable licensure, certification, and scope-of-practice requirements.
- Coordinate care with physicians and other members of the interdisciplinary care team.
- Maintain accurate and timely clinical documentation and coding in accordance with applicable requirements.
- Provide culturally responsive, compassionate, and equitable care to patients from diverse backgrounds and populations.
- Participate in the Patient-Centered Medical Home and Team-Based Care model.
- Maintain patient confidentiality and comply with applicable privacy, regulatory, and organizational requirements.
- Participate in quality improvement initiatives and other activities that support CHA's mission and commitment to high-quality care.
- Depending on location and operational needs, obtain and maintain a State Pharmacy Dispensing License and dispense medications.
Knowledge, Skills & Abilities
The successful candidate will demonstrate:
- Knowledge of primary care and family medicine principles, practices, and procedures.
- Knowledge of the diagnosis and treatment of a broad range of acute and chronic diseases and injuries.

- Knowledge of applicable CPT and ICD-10 coding practices.

- Ability to operate standard medical equipment, including EKG machines, pulse oximeters, nebulizers, spirometers, audiology equipment, sterilizers, and other equipment appropriate to the practice setting.
- Ability to read, write legibly, perform mathematical calculations, and accurately document clinical information.
- Ability to solve practical problems and respond effectively to a variety of clinical and operational situations.
- Ability to perform the essential physical functions of the position, with reasonable accommodation as appropriate.
Education, Experience & Licensure
Required:
- Graduation from an accredited Physician Assistant or Advanced Practice Registered Nurse program appropriate to the provider's professional designation.
- Current certification and licensure as a Physician Assistant or Advanced Practice Registered Nurse in the State of Nevada.
- Current national certification appropriate to the provider's professional designation.
- Ability to obtain and maintain any additional licenses, registrations, certifications, or credentials required for the position and specific health center location.
Preferred:
- 1–2 years of experience in a community health center, federally qualified health center (FQHC), rural health setting, or similar primary care environment.
- Experience providing comprehensive family medicine or primary care services.
