Population Health Outreach Specialist

The Population Health Outreach Specialist’s primary objective is to work with the Population Health team on value-based program outreach projects. This role is dedicated to strategic outreach efforts prioritized to address screening/preventative gap closure needs which align with value-based partnerships. It involves organizing, engaging, and tracking outreach efforts that promote health and wellness in the community. This role manages the workflow and support of primary care provider (PCP) assignment/reassignment with health plan partners. This position must demonstrate exceptional customer service skills and professionalism. This position is required to be proficient in reading/writing/speaking Spanish.

Required Knowledge, Skills & Abilities:

•  Exceptional customer service skills, patient, and professionalism

•  Knowledge of Electronic Medical Records (EMR) system scheduling and patient chart navigation or ability to quickly learn
•  Demonstrates experience managing tracking reports (Excel spreadsheets) 
•  Skilled with Excel and Microsoft Office
•  Ability to summarize reporting information
•  Knowledge of medical terminology
•  Ability to work and interact effectively and positively with other staff members to build and enhance teamwork

•  Ability to maintain confidentiality of financial and medical information according to Health Insurance Portability and Accountability Act (HIPAA) regulations

•  Ability to read, write legibly and calculate mathematical figures
•  Ability to hear and speak to converse over the telephone and in-person in a professional and respectful manner, in both English and Spanish
•  Can operate various office equipment, such as calculator, copy machine, fax, smart phone, and telephone system

Education and Experience: High School Diploma or GED required. One year of relevant experience is preferred. Must be able to read, write, and speak Spanish fluently. Experience with healthcare related outreach calls, managing/tracking healthcare related data, and/or experience with an Electronic Medical Records (EMR) system preferred.
