    Urgent Care Provider (APRN or PA) - CHC of Stamford

    This is a full time position supporting our CHC location in Stamford, CT. Join our primary care team in delivering full primary care and urgent care services in a patient centered medical home.

    As a Urgent Care Provider for Community Health Center, you will:

    • Assess and treat common acute and episodic illness in patients of all ages.
    • Provide preventative services as appropriate
    • Perform appropriate diagnostic waived testing as required and arrange for follow up care with PCP as indicated
    • Provide problem-specific education to individual and family members as appropriate
    • Utilize evidence based standards in formulating treatment plan, and communicate rationale for decision making with patients
    • Document all findings, including appropriate plan for follow up care, in electronic health record
    • Participate in the organization’s quality improvement program

    BASE COMPENSATION RANGES

    • Physician: MD/DO from approved medical/osteopathy school.  Graduate of an accredited residency program in family practice, internal medicine, pediatrics, or OB/GYN. Board certified within 24 months of commencing employment at Community Health Center. Pay Range: $200,000 - $280,000**
    • APRN (Advanced Practice Registered Nurse): Graduate of an accredited Master of Science in Nursing program with a specialty as a family nurse practitioner.  Maintains board certification as a family nurse practitioner.   Maintains appropriate board certification, licensure, and professional organizations. Pay Range: $120,000 - $170,000**
    • PA (Physician Assistant): Graduate of an accredited Master program in the study of physician assistant. Maintains appropriate board certification, licensure, and professional organizations. Pay Range: $120,000 - $170,000**

    ** This pay range represents both the base hourly rate or base annual full-time salary, as well as the top hourly rate or annual full time salary for all positions which this job grade or position falls within. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.

    QUALITFICATIONS AND PREFERRED SKILLS

    • CHC requires as a condition of employment current American Red Cross CPR for the Professional Rescuer and AED (CPR/FPR/AED) certification. The only acceptable alternative is current American Heart Association BLS/AED for Healthcare Providers certification.
    • Communicates effectively with clients and staff.  Documents interaction clearly and concisely in the medical record. Requires effective communication skills at all levels of interaction.  Required to develop bilingual capacity as dictated by needs of the site.

    PHYSICAL REQUIREMENTS/WORK ENVIRONMENT

    • Moderate physical activity level, i.e. walking, standing.  Potential blood born pathogen/chemical hazard exposure.

    WORK SCHEDULE DEMANDS

    • Hours as agreed.  Clinical schedule arranged to include office hours, hospital rounds, etc. One evening office session per week. and one weekend session
