Care Coordinator

Hours: 40 hours per week. Work schedule is Monday, Tuesday, Wednesday, and Friday, 8:30 am to 5:00 pm or 9:00 am to 5:30 pm; Thursday, 11:30 am to 8:00 pm

As a Care Coordinator, you will:

• Conduct initial assessment of case management needs for all new and returning clients.
• Provide care coordination cor clients and family members to support the client, including psychosocial rehabilitation and recovery principles.
• Conduct safety and risk screenings when concerns arise; work closely with program team regarding any critical incidents or concerns to ensure client needs are addressed and to ensure optimum outcomes.
• Develop and maintain professional working relationships with community health, welfare, and social service agencies.  Coordinate with community partners to identify resources, services, and entitlements appropriate for clients.

Our Adult Outpatient Programs (Areta Crowell Wellness Recovery Center, Douglas Young Wellness Recovery Center, Heartland Wellness Recovery Center, Jane Westin Center, Maria Sardinas Wellness Recovery Center, and South Bay Guidance Center) provide behavioral health services including psychiatric and clinical assessment, treatment planning, ongoing medication services, individual and group counseling, case management, crisis intervention, mobile outreach, vocational support, and peer support. We deliver fully integrated services to adult clients diagnosed with a serious mental illness, as well as individuals with co-occurring, mental health, and substance use disorders.

All of our outpatient programs also provide full-time walk-in services. Our programs utilize evidence-based practices to assist our clients in reaching their goals and to live the highest quality of life while managing their mental illness.

CRF staff members work together to meet the needs of San Diego County's residents who are experiencing mental/behavioral health challenges. Education:  Bachelor's Degree in related field such as Psychology, Social Work, or Counseling is required

Additional Qualifications and Skills

• Minimum of 1 year of experience working with a mental health population, especially with individuals with severe and persistent mental illness and co-occurring disorders is strongly preferred
• First Aid/CPR certification strongly preferred; required post hire and must be kept current throughout employment
• Proficient computer skills, including MS Office
• Ability to thrive in a fast-paced environment is essential
• A valid California driver’s license in good standing, personal vehicle, proof of insurance and eligibility to be an approved CRF driver is required to fulfill the essential functions of this position
• This program is housed in a two-story building that does not have an elevator. Ability to navigate stairs is required.
