    Job description

    Provide hands-on personal care and assist with self-care skills to clients in their homes.

    • Develop a successful interpersonal relationship with our clients.
    • Assist our clients with activities of daily living, including bathing, light housekeeping, feeding and preparing meals.
    • Follow infection control and safety measures when caring for clients.
    • Recognize and report changes in client's condition and safety to care coordinators
    • Positions Available:
    • Days 7am to 7pm Saturday Sunday
    • Nights 11 pm to 7 am. Saturday and Sunday
    • Days 7 am to 7 pm Monday through Thursday
    • At least one year of professional experience as a Caregiver and completion of an CNA course
    • Licensed CNA in the state of Illinois
    • Two forms of ID such as Driver's License, State ID, Passport, Social Security Card
    • General Physical Examination and TB Test within last year or Chest X-Ray
    • CPR Certification
    • Two Professional References

We provide home health care under Medicare and Private Insurance to patients in their home; be it a single-family home, apartment building or assisted living. Our Registered Nurses, Therapists, and Social Worker coordinate care in Lake & McHenry County in Illinois and Kenosha, Racine & Walworth County in Wisconsin. We work with hospitals, skilled nursing providers and primary physicians to continuously assess medical conditions; ensure medication compliance by educating patients and caregivers on the purpose, frequency, proper administration, and possible side effects of medications; all while maintaining a level of care, compassion and consistency that dramatically improves the quality of life of our patients.
