Full time – 40 hours per week
SALARY RANGE:
31,824 - 45,760 GENERAL STATEMENT OF
DUTIES:
- To provide effective community-based coordination and assistance in accessing appropriate services that address the individualized mental health needs of the client.
- They are directed towards adults, children, adolescents and families and will vary with respect to hours, type and intensity of services, depending on the changing needs of each individual .
- ESSENTIAL
RESPONSIBILITIES:
- 1.
- Provide specific, measurable, and individualized services to severely mentally disabled or seriously emotionally disturbed client, their families and other individuals.
- 2.
- Provide services that are focused on the individual's ability to succeed in the community; to identify and access needed services; and to show improvement in school, work and family and integration and contributions within the community.
- 3.
- Provide ongoing assessment of needs and assistance in achieving personal independence in managing basic needs.
- 4.
- Facilitation of further development of daily living skills.
- 5.
- Coordination of the Individual Service Plan.
- 6.
- Symptom monitoring and assistance and coordination in crisis management and stabilization.
- 7.
- Mental health interventions that address symptoms, behaviors, thought processes, etc., that assist an individual in eliminating barriers to seeking or maintaining education and employment.
- 8.
- Conducting activities that increase the individual's capacity to positively impact his/her own environment.
- 9.
- At a minimum, achieve and maintain the Agency’s established productivity standards.
- 10.
- Maintain current knowledge of all applicable F&CS policies and procedures and function according to these standards.
- 11.
- Be knowledgeable of and adhere to all pertinent federal, state, and local laws, standards of OMHAS, and appropriate professional ethical standards.
- 12.
- Comply with COA regulations and educational/certification/registry requirements.
- 13.
- Other duties as assigned.
