NURSE PRACTITIONER
SUMMARY:
As part of a collaborative agreement with a physician as mandated by Ohio law and under the direction of the AxessPointe Community Health Center, Inc. (API) Medical Director, the NP/PA will provide quality primary medical and behavioral health services to adults, children and families in a holistic manner within the northeast Ohio area. SUPERVISOR: Medical Director or Designee
DUTIES AND RESPONSIBILITIES:
- Examines, diagnoses, and treats illnesses and injuries of patients throughout the lifespan.
- Provides well-child, well-woman and well-man health maintenance exams to new and established patients.
- Orders appropriate screening and diagnostic lab and radiographic exams.
- Interprets results of physical exams and screening/diagnostic tests to implement appropriate care plans for the patient.
- Prescribes medicines, therapeutic agents and treatment modalities as required by patient needs and in accordance with licensed prescriptive authority.
- Supports health promotion, health education, health empowerment and disease prevention interventions for all patients.
- Provides age-appropriate care to all patients.
- Promotes patient and family centered care through a collaborative team approach.
- Participates in established call group as necessary.
- Refers patients to specialty practices as appropriate and needed.
- Provides medical care to patient as part of mutual patient care with specialist.
- Confers with collaborating physician about patient care as necessary.
- Documents and maintains patient records of services provided according to program standards.
- May instruct medical students or nursing students.
- Operates within a scope of practice as agreed to by the FNP/PA, the collaborating physician, the medical director and in compliance with Ohio state law.
