What You'll Do As an ACT Team LPN, you'll provide nursing care, care coordination, and support to adults receiving community-based behavioral health services. Working alongside therapists, case managers, peer support specialists, prescribers, and other professionals, you'll help clients remain healthy, stable, and connected to the care they need.

Primary Responsibilities

• Provide nursing care in clients' homes and other community settings, including: Nursing assessments Medication administration Medication education Health monitoring Coordinate care with psychiatrists, primary care providers, hospitals, pharmacies, and community partners to ensure continuity of care.
• Serve as an advocate for clients by helping them access medical, behavioral health, and community resources.
• Educate clients and their families about medications, physical health, mental health conditions, and wellness strategies.
• Assist with crisis intervention and emergency response when needed by coordinating with crisis services, emergency responders, supervisors, and medical providers.
• Participate as an active member of the ACT team, attending team meetings and collaborating to develop individualized treatment plans.
• Help clients develop skills to improve independence and manage emotional distress.
• Build trusting, recovery-focused relationships with clients and their families.
• Complete documentation accurately and in a timely manner.
• Transport clients and provide services throughout the community as needed.
