POSITION SUMMARY:

The Case Manager will serve and provide support to on a daily basis. This person should be focused and motivated, while working with facility management to handle client-related issues. Job Type: Full Time Sarting Pay: $32 Work

Location:

Merced RESPONSIBILITIES · Plan monthly in-house and offsite activities for clients. · Transport clients to and from scheduled appointments. · Supports and assist individuals in developing independent living skills and meeting rehabilitation goals. · Conduct group and 1x1 sessions with clients. · Oversee NSP resident goal, placement, and discharge. · Meet with county case manager and conservators as needed. PHYSICAL REQUIREMENTS · Ability to climb stairs multiple times a day and be able to lift and/or carry up to 25 pounds. · Ability to sit, rise, kneel, squat, twist, pull bend multiple times a day. · Ability to walk with items in hand, grasp and reach as needed. · Ability to perform with repetitive maneuvers and manual dexterity. · Ability to work in an office environment.
