Job Responsibilities:
- Perform ongoing clinical assessments to determine patient needs.
- Evaluate patients’ occupational performance in self-care, work, leisure, and rest.
- Assess and facilitate therapeutic interventions for ROM, strength, coordination, adaptive equipment, and functional mobility.
- Develop and evaluate patient care plans in collaboration with patients, caregivers, and representatives.
- Document clinical notes, phone orders, and other records promptly and accurately.
- Provide education and counseling to patients and caregivers.
- Supervise and guide occupational therapy assistants and home health aides as necessary.
- Apply infection control and standard precautions during patient care activities.
- Communicate effectively with physicians and healthcare practitioners involved in the patient’s care.
- Meet continuing education requirements and demonstrate professional growth and competency.
- Promote the Agency’s philosophy and policies to ensure high-quality care.
Environmental and Working Conditions:
- Work in patients’ homes under various conditions, with possible exposure to blood, bodily fluids, and infectious diseases.
- Ability to work a flexible schedule and travel locally, with occasional exposure to unpleasant weather.
