Job Title: Home Health Registered Nurse (RN) – Case Manager Location: South Bay, CA Job Type: Full-Time
Health Link Home Health & Hospice is a trusted provider across the Bay Area for nearly 20 years. We are currently seeking a compassionate and organized Registered Nurse (RN) to join our South Bay team.

Ride share Support (SF Only): We know parking in the city is a challenge. We provide rideshare credits (Uber/Lyft) to get you to and from patient visits so you don’t have to battle for parking.

Smart Documentation: We use AI-assisted charting software (Kinnser) to drastically reduce your documentation time.
Real-Time Support: You are never alone. Our clinical leadership and intake teams are available for real-time field support when you need orders or coordination.
Target Compensation: $135,000 – $156,000 per year

Manage Your Caseload: Perform comprehensive nursing assessments (OASIS), including Start of Care, Recertifications, and Discharges.
Skilled Care: Administer medications, provide wound care (including Wound Vacs), and manage IV therapies.
Coordinate: Act as the primary point of contact for physicians and the interdisciplinary team (PT/OT/ST) to ensure the care plan is working.
Educate: Empower patients and families to manage their conditions safely at home.
Document: Maintain accurate, timely clinical notes on company-issued iPads.
LICENSE: Current, active California RN License is REQUIRED to apply. Applications without a valid CA license number will not be reviewed.
Experience: Minimum 1 year of professional nursing experience required. Home Health experience is preferred, but we are willing to train strong hospital/SNF nurses.
Travel: Valid California Driver’s License and reliable transportation are mandatory.
Skills: Strong clinical judgment, organization, and the ability to work independently.
