Job description

Perform true case management functions, interacting with patient and managing their clinical care telephonically
Work with patients/members in the foster care program
You will be responsible for developing and assessing the care plan and promote desired outcomes
Provider patient and provider education, and more!

RN license clear and active in the state of Washington
Case Management experience
Pediatrics experience preferred
2+ years of acute/inpatient hospital experience
Strong Computer Skills
