In-Home Mental Health Clinician, IIH-C
Full-Time and Part-Time | Monmouth & Middlesex Counties, NJ
Full-time salary: $85,000 annually
Part-time pay: $45 per hour
Schedule: Primarily weekdays, 3:00–8:00 PM, and weekends, subject to client availability

Heart to Heart is hiring clinicians for our Intensive In-Home Clinical (IIH-C) program in Monmouth and Middlesex Counties. You will provide individualized therapy and clinical support to children, adolescents, and young adults under age 21 with intellectual/developmental disabilities, including autism, and emotional, behavioral, or mental health needs.
This role offers meaningful, direct work with youth and caregivers in their homes and communities. You will help families strengthen relationships, manage challenges, and support their child’s ability to remain successful at home, in school, and in the community.
Qualifications
A master’s degree in social work, counseling, psychology, special education, or a related field that meets IIH-C requirements.
An active New Jersey independent clinical license, such as LCSW, LPC, or LMFT; or eligibility to obtain independent New Jersey clinical licensure within two years while practicing under the required supervision.
At least one year of experience providing clinical services to individuals with intellectual/developmental disabilities.
Ability to build supportive relationships with youth and caregivers, adapt interventions to developmental needs, and remain calm when situations become challenging.
Strong clinical judgment, communication, organization, and documentation skills.
Reliable transportation and the ability to travel between client homes and community locations within the assigned service area.
Availability for afternoon, evening, and weekend appointments.
Ability to meet required background screening, health screening, credentialing, and program training requirements.
Experience with CSOC, CYBER, family therapy, autism, positive behavioral supports, or the Nurtured Heart Approach is a plus. Bilingual clinicians and applicants with NADD or DIR/Floortime training are encouraged to apply.
Schedule and Work Setting
Services are delivered in person in client homes and community settings throughout Monmouth and Middlesex Counties. Travel between appointments is part of the position.
Client appointments will primarily occur between 3:00 and 8:00 PM on weekdays and on weekends, with scheduling based on client and family availability. Full-time responsibilities may also include assessments, documentation, care coordination, and team meetings outside direct treatment sessions.
Part-time applicants should share their available days, hours, and preferred service area.
