    Role Overview

    As a Case Manager, you'll be a vital member of our patient-centered care team, ensuring seamless coordination and advocacy for patients and their families. Your role is to identify, assess, and plan care strategies that promote safety, quality, and cost-effective outcomes. You'll facilitate transitions to the next level of care, manage critical diagnoses, and optimize health outcomes across the care continuum, making a direct impact on patient wellbeing.

    What You Will Need

    Required Qualifications

    • Bachelor¿s degree in nursing or Master¿s degree in Social Work.
    • Minimum 3 years of clinical experience.
    • Valid, unrestricted State of Michigan license as a Registered Nurse (RN) or Licensed Social Worker (LMSW).
    • Excellent verbal communication, written documentation, and interpersonal skills.
    • Strong problem-solving, analytical, and decision-making abilities.
    • Proficiency in computer skills and knowledge of healthcare systems.

    Preferred Qualifications

    • Certification in Case Management (CCM) or Accredited Case Manager (ACM).
    • Experience in discharge planning, home health care, rehabilitative medicine, or managed care.
