As part of Henry Ford Health's Community Care Services, our Home Health Care team plays a vital role in expanding access to care by delivering essential health services directly to patients in their homes. We focus on meeting individuals where they are, providing care coordination, and connecting them with resources such as physical therapy, occupational therapy, and social work. Our mission is to make a meaningful impact across diverse populations, ensuring integrated and compassionate care for those we serve.
Role Overview
As an RN in Home Health Care, you will independently provide direct patient care in the home, utilizing your nursing expertise to assess and address patient needs. You will coordinate with allied health professionals to ensure a comprehensive care plan, supporting patients and their families in achieving optimal health outcomes. Your role is critical in delivering personalized, high-quality care that improves quality of life.
