    East Side Detroit & St. Clair Shores Territory

    $10,000 Sign-On/Retention Bonus

    This position covers the East Side region, including:

    St. Clair Shores: 48080, 48081, 48082

    Detroit: ZIP Codes: 48205, 48207, 48211,48213,48214,48215,48224,48234

    Additional Coverage Areas: Eastpointe (48021), Roseville (48066), Harper Woods (48225), Hamtramck (48212), Grosse Pointe City, Grosse Pointe Park, Grosse Pointe Farms, Grosse Pointe Shores; Grosse Pointe Woods

    Candidates should be comfortable with daily driving throughout the territory.

    As a Home Health RN, you'll never have the same day twice. Our nurses care for patients with a wide range of needs and conditions, including:

    • Wound care and wound vac management
    • Diabetes education and management
    • Feeding tubes and other specialized treatments
    • Chronic disease management
    • Medication teaching and compliance
    • Post-acute and complex medical patients
    • Comprehensive patient and caregiver education

    You'll serve as both a skilled clinician and case manager, coordinating care and helping patients achieve the best possible outcomes at home.

    Position Details

    • Full-Time and Part-Time openings available
    • Day Shift
    • Flexible scheduling between 8:00 AM - 4:30 PM
    • Monday through Friday
    • Salary-based position
    • Additional earnings through a 6-point productivity system
    • Manage your own daily schedule
    • Charting required within 24 hours of patient visits
    • Significant local travel within assigned territory
    • One weekend per month
    • Holiday rotation: 1 summer holiday and 1 winter holiday annually

    Role Overview

    As an RN in Home Health Care, you will independently provide direct patient care in the home, utilizing your nursing expertise to assess and address patient needs. You will coordinate with allied health professionals to ensure a comprehensive and integrated care delivery system, supporting patients and their families in achieving optimal health outcomes.

    What You Will Do

    • Provide direct patient care in the home, applying nursing principles to assess and meet patient needs.
    • Coordinate with allied health professionals to ensure integrated care delivery.
    • Document patient care electronically using computer systems.

    • Transport and move clients as needed to ensure safe and effective care.

    What You Will Need

    Required Qualifications

    • Graduation from an accredited Registered Nurse program.
    • Current licensure as a Registered Nurse in the State of Michigan.
    • Ability to effectively communicate with patients, families, and healthcare personnel.
    • Knowledge of computer use for electronic documentation.
    • Current driver's license, satisfactory driving record, and reliable transportation.
    • Ability to lift, carry, transport, and move clients as needed.

    Preferred Qualifications

    • Minimum of nine months to one year of clinical nursing experience.
