Responsibilities
Provide direct patient care to residents in community or home settings, including basic life support, hygiene, mobility assistance, and daily living activities.
Monitor patient condition, report changes, and document in EHR systems accurately.
Assist with feeding, medication reminders, and coordination with nursing staff and supervisors.
Ensure safety through proper body mechanics, fall prevention, and adherence to care plans.
Support geriatric and memory care needs, including Alzheimer's and long-term care considerations.
Qualifications
CNA with certification and current license
Basic life support, patient care, and manual handling skills
Experience in home/community care and geriatric settings
Proficiency with EHR systems
Knowledge of physiology and nursing home care; ability to support memory care and senior patients
Strong patient service orientation and ability to work in long-term or post-acute care environments
