    Hope Community Resources, Inc. is a non-profit disabilities service provider based in Anchorage, Alaska. Founded in 1968, our company provides services and supports to people of all ages and abilities who experience an intellectual, developmental, or other disability, traumatic brain injury, or mental health challenge.

    As a Mental Health Case Manager, you will provide individualized support to people experiencing mental health challenges. You will help support recipients identify needs, access resources, coordinate services, and navigate systems that can impact their health, stability, independence, and quality of life.

    This position works closely with support recipients, families, service providers, and community partners to coordinate care and ensure individuals have access to the services and resources they need.

    Hope's Mental Health Case Managers provide:

    • Community Recovery Support Services (CRSS)

    • Case Management (CM)

    • Intensive Case Management (ICM)

    Experience and Education: 

    • Associate degree in Health Science, Social Science, Human Services, Psychology, or related field; or,
    • Two years of experience working directly with individuals who experience a mental illness or behavioral health challenges; or,
    • One year of experience providing direct case management services including extensive coordination of services, collaborating with extra-agency treatment team members, and provision of direct treatment services; or,
    • Equivalent combination of related education and/or work experience.
