ADVANCED PRACTICE PROVIDER FLOAT- FAMILY MEDICINE

Horizon Health Float Advance Practice Provider for Family Medicine and Specialty Services.

Horizon Health has an opportunity for a float Advance Practice Provider to cover Family Medicine with the potential to work in other specialty areas in the future.

We are a rural organization that includes a critical access hospital, extensive outpatient services, and rural health clinics, but much larger in scope and size than typical rural organizations. We have advanced technology, new offerings for the communities we serve, strong specialty support, and still have the small-town approach and values that make our organization so special.

The Advance Practice Provider (APP) will initially provide primary care for our patients in outlying clinics. We are looking for an APP candidate with experience, compassion, and strong clinical skills. This APP will assess patients regarding their conditions, order and interpret diagnostic tests, and work with patients and families to establish care plans.

Family Practice Information: 

• Current license as an Advanced Practice Registered Nurse in the State of Illinois. Willing to get Indiana licensure.
• Cardio-Pulmonary Resuscitation-BLS.
• Preferred three (3) years of Advance Practice Provider experience.
• Compassionate patient centered approach to care
• Flexible; willing to work at any of our locations if needed; Marshall, Paris, Tilton, Chrisman, Oakland locations in Illinois and Terre Haute in Indiana.
• Excellent time management skills
• Exceptional communication skills
• Ability to work independently
• Horizon Health is staffed with 21 primary care physicians and providers, with specialty support in behavioral health, urology, women’s health, general surgery, orthopedics, spine, pain, dermatology, podiatry, wound care, neurology, endocrinology, and cardiology.
• Additionally, we have 13 visiting specialists that see patients in the clinic each month. The hospital has a 24/7 hospitalist service and convenient care for patients.
• We have a physician friendly EMR.
• In 2024/2025, we completed expansion projects including ED expansion, Tilton Primary Care Clinic, Marshall Primary Care Clinic, and Physical Therapy expansion into Terre Haute, IN.
