    Position Summary:

    Our dedicated Chronic Care nursing team make a lasting difference in the lives of patients with complex, long-term medical needs. Here, you will build meaningful relationships and provide compassionate, holistic care that promotes independence and quality of life. As part of the Hospital for Special Care community, you will have the opportunity to expand your skillset and explore other specialty areas across the hospital as your career grows.

    Key Responsibilities:

    • Provide compassionate, evidence-based nursing care to patients in a long-term acute care or rehabilitation setting.
    • Collaborate with physicians, therapists, and other care team members to develop and implement individualized care plans.
    • Monitor, assess, and document patient progress while ensuring safety and comfort.
    • Educate patients and families on treatment plans, medication management, and discharge planning.
    • Support a culture of excellence, teamwork, and continuous learning.

    Qualifications:

    • Required: Current Registered Nurse (RN) license in the State of Connecticut.
    • Required: Graduate of an accredited school of nursing (BSN preferred).
    • Required: Demonstrated competence in the clinical setting.

    COMPENSATION & BENEFITS
    At Hospital for Special Care, we are committed to supporting the health, financial well-being, and professional growth of our team members. The wage range for this position is $38.50 to $57.82.
