POSITION SUMMARY:
Under the direction of the Program Manager of the H.I.P.P program, the Patient Navigator is responsible for conducting outreach and inreach activities, assist with intakes and provide education to potential PEP/PrEP and HIV positive newly diagnosed patients seeking services. The Patient Navigator also assist in the coordination of medical appointments, troubleshoots patient insurance issues; enters data into AIRS, conducts follow up with clients, and provides retention and adherence counseling to PEP/PrEP and primary care patients.
