We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our growing healthcare team. The primary focus of the RN Case Manager is to assess, plan, implement, and evaluate patient care to ensure optimal outcomes. This position collaborates with healthcare providers, patients, families, and other team members to develop and monitor care plans, provide patient education, and promote quality, cost-effective healthcare services.
Responsibilities
Associate level medical/nursing or equivalent.
Completion of an accredited RN program. Current and valid Oklahoma RN license.
3+ years of clinical experience preferred.
BLS CPR Certification.
Critical thinking skills, prioritization, ability to adapt and take constructive criticism, and frequent changes is tasks with ability to complete in a timely manner.
Medical terminology, communicate medical results to patients in a way they can understand, and have a good understanding of triage medicine and pathophysiology.
Basic computer skills and able to navigate multiple medical records systems.
Experience with RPMS or willingness to learn.
