This LPN position will provide care for clients in their homes, healthcare provider offices, on the mobile health unit, WIC clinic, public health offices, and more. This position is supporting mobile integrated health, decreasing transportation barriers for clients.

Responsibilities:
The Licensed Practical Nurse (LPN) supports the Mobile Integrated Health (MIH) Program and GROW Initiative by providing community-based maternal and infant health services to pregnant individuals, postpartum mothers, infants, and women of reproductive age. This position focuses on improving access to care, reducing transportation barriers, addressing social drivers of health, and connecting families to healthcare services and community resources through outreach, education, navigation, and clinical support. The role supports maternal and infant health outcomes through home visits, community-based interventions, care coordination, and collaboration with healthcare providers and community partners. The position aligns with regional efforts to expand maternal health navigation, community outreach, telehealth utilization, and coordinated care services.
