Position Summary
The Family Success Specialist serves as the primary relationship manager and point of contact for assigned families within Jack & Jill Center's Family Success System. Using a strengths-based, trauma-informed, and culturally responsive approach, the FSS conducts family assessments, develops individualized Family Success Plans, connects families to wraparound services and community resources, and walks alongside families as they pursue self-defined goals across eight goal domains.
Primary Position Responsibilities
Family Success Planning & Service Coordination
- Serve as the primary relationship manager and point of contact for assigned families.
- Conduct strengths-based intake interviews, family assessments, and annual reassessments using a person-in-environment lens.
- Develop individualized Family Success Plans with measurable goals and action steps.
- Maintain regular contact with families through meetings, phone calls, home visits, virtual appointments, and follow-up communications.
- Identify strengths, barriers, risks, and opportunities that impact family success.
- Connect families to community resources and coordinate referrals with follow-up to ensure linkage and engagement.
- Monitor progress toward family goals and adjust service plans as needed.
- Guide families through the goal-setting process using established goal domains, letting families lead the conversation while organizing services and outcomes consistently behind the scenes.
- Maintain documentation in appropriate systems (e.g., Blackbaud, Apricot, and other third-party platforms), ensuring that records remain accurate, complete, and up to date across all systems.
Enrollment & Family Transition Support
- Support family enrollment, re-enrollment, and retention efforts by helping families navigate requirements, timelines, and barriers that may impact continued enrollment.
- Proactively engage families identified as at risk of not enrolling or returning to understand concerns, identify barriers, and coordinate appropriate supports or resources.
- Conduct follow-up with families who withdraw or do not return to understand and accurately document the reason for departure and identify opportunities for re-engagement when appropriate.
- Collaborate with Education, Admissions, and Family Strengthening leadership to support enrollment outreach, family transitions, and continuity of services.
- Maintain accurate enrollment, withdrawal, and family transition information in designated systems and help identify trends or recurring barriers impacting enrollment and retention.
Family Stability & Crisis Support
- Assist families in addressing immediate and long-term needs related to housing, food security, transportation, childcare, benefits access, and financial stability.
- Provide crisis intervention, support, and safety planning during family emergencies.
- Escalate high-risk situations, child welfare concerns, and complex family issues to the Family Strengthening Manager.
- Coordinate holiday assistance, food distributions, and donations in alignment with centralized departmental guidelines to ensure equitable allocation and reduce duplication.
Economic Mobility & Educational Advancement
- Support caregivers in pursuing educational advancement, workforce development, vocational training, certifications, and higher education opportunities.
- Provide employment readiness coaching, including resume development, interview preparation, and job search support.
- Connect families to workforce development programs, educational institutions, and community partners.
Parent Education & Family Engagement
- Coordinate and assist with facilitation of evidence-based parenting education programs, classes, and workshops aligned with family goals.
- Recruit, register, and engage parents in educational opportunities; track attendance and follow up regarding participation and completion.
- Assist with parent nights, workshops, family events, and special initiatives.
Health & Wellness Coordination
- Collaborate with the School Nurse and other health professionals to support family health and wellness goals.
- Connect families to healthcare providers, telehealth services, insurance resources, and community wellness programs.
- Connect families to mental health providers and behavioral health services, recognizing that mental health supports are a critical gap for many families experiencing economic hardship and related stress.
Outcome Measurement & Documentation
- Track and document family progress toward established outcomes and goals.
- Collect and maintain accurate program data and required documentation for grant reporting.
- Participate in protected documentation time as established by the Family Strengthening Manager to ensure case notes, assessments, and outcome data are completed accurately and on time.
Alumni & Transition Support
- Support graduating families through the transition process, including scholarship guidance, closing assessments, and warm handoffs to alumni services.
- Maintain alumni outreach cadence (semiannual or annual) and re-engage alumni families who need services.
Community Partnership & Outreach
- Develop and maintain positive partnerships with community organizations and service providers.
- Maintain current knowledge of available community resources and referral opportunities.
- Represent Jack & Jill Center at community meetings, events, and networking opportunities

The Family Success Specialist possesses the following minimum qualifications:
Required:
- Bachelor's degree in Social Work, Human Services, Psychology, or related field.
- Minimum 2 years of experience in case management, family services, or social services.
- Knowledge of strengths-based practice, trauma-informed care, and cultural humility principles.
- Proficiency in case management databases and data entry.
- A valid driver's license and access to reliable transportation.
- Proper documentation to work in the United States.
- Successfully complete and pass a Level II Clearinghouse Background Screening - FL Clearinghouse | Florida Agency for Health Care Administration
Preferred:
- Master's degree in Social Work (MSW) or related field.
- Bilingual (English/Spanish or English/Creole)
- Experience with Two-Generation (2Gen) service models.
