    Job description

    The Home Health Assistant Clinical Director will play a key role in ensuring compliance, enhancing staff performance, and promoting patient-centered care. The home Health Assistant Clinical Director will support the clinical Director in overseeing clinical operations and delivering high-quality home health care.

    Schedule: Full-Time, Weekday (M-F), 9AM - 5PM

    Primary Responsibilities and Duties:

    • Assist the Clinical Director in overseeing daily operations of the home health agency, ensuring quality care and regulatory compliance.
    • Support the management of clinical staff, providing mentorship, training, and guidance in best practices for patient care.
    • Collaborate with medical directors, nursing staff, clinical director, and other health care team members to develop and evaluate patient care plans.
    • Conduct regular clinical audits and reviews to ensure adherence to quality standards and best practices.
    • Assist in the development and implementation of clinical policies and procedures to enhance service delivery.
    • Provide ongoing education and training for clinical staff on new policies, procedures, and care techniques.
    • Act as a liaison between patients, families, and the clinical team, addressing concerns and facilitating communication.
    • Stay current with industry trends, regulations, and advancements with hospice care
    • Ensure the clinical team is accurately documenting care provided and reporting any significant changes in the patient’s condition. This includes noting exact medication and care provided with the date and time for insurance purposes and appropriate quality of care to patients.
    • Adhere to all safety protocols and infection control measures.
    • Collaborate with family members and health care teams to ensure continuity of care.
    • Adhere to infection control and safety procedures to protect patients and staff.
    • Perform all other duties assigned.
