The Physical Therapist will play a crucial role in providing high-quality rehabilitation services to our residents, with a focus on restoring mobility, improving strength and function, and enhancing overall quality of life.
Schedule: Part-Time, PRN, Weekdays, Weekends, On-Call
Primary Responsibilities and Duties:
- Evaluate Residents' physical abilities, functional status, and rehabilitation needs through comprehensive assessments.
- Develop personalized treatment plans based on assessment findings, resident goals, and evidence-based practice guidelines.
- Implement therapeutic interventions to address impairments, maximize functional independence, and promote safe mobility.
- Provide one-on-one physical therapy sessions to residents, utilizing a variety of therapeutic techniques and modalities.
- Monitor resident progress and adjust treatment plans as needed to optimize outcomes and achieve rehabilitation goals.
- Collaborate with team members, including physicians, nurses, occupational therapists, and speech-language pathologist, to coordinate care and ensure continuity of services.
- Educate residents and their families on exercise programs, safety techniques, and home exercise regimens to support ongoing rehabilitation and prevent re-injury.
- Document all physical therapy services provided, including initial evaluations, progress notes, and discharge summaries, in accordance with facility policies and regulatory requirements.
- Participate in resident care conferences, team meetings, and in-service training sessions to share expertise, contribute to care planning, and stay informed about best practices in rehabilitation.
- Contribute to a positive and supportive work environment by demonstrating professionalism, teamwork, and effective communication skills.
- Perform all other duties assigned.
