    Job description

    The Home Health Occupational Therapist provides expert therapeutic services to patients in their homes, focusing on enhancing their ability to perform daily activities and improving their overall quality of life. In this role you will assess, plan, and implement individualized therapy programs in collaboration with patients, families, and other healthcare professionals to address physical, cognitive, and emotional challenges.

    Duties:

    • Conduct comprehensive assessments of patients' physical, cognitive, and emotional needs to develop personalized therapy plans.
    • Design and implement individualized therapy programs to address impairments, functional limitations, and performance deficits.
    • Educate patients and their families on techniques and strategies to improve daily living skills and adapt their environment to support functional independence.
    • Utilize therapeutic interventions, including exercises, adaptive equipment, and modifications to daily activities, to enhance patients’ abilities and quality of life.
    • Monitor and document patient progress, response to therapy, and any changes in condition, providing detailed reports to the supervising physician and other healthcare team members.
    • Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure a holistic and coordinated approach to patient care.
    • Provide guidance on the use of assistive devices and adaptive technologies to support patients’ functional abilities.
    • Maintain accurate and up-to-date patient records and documentation in compliance with regulatory and organizational standards.
    • Offer support and education to caregivers and family members to improve patient care and facilitate the therapy process.
    • Participate in ongoing professional development and stay current with advancements in occupational therapy practices and techniques.
    • Perform all other duties assigned.
