The Home Health Speech Therapist provides specialized speech and language therapy services to patients in their homes. In this role you will be responsible for evaluating, diagnosing, and treating speech, language, communication, and swallowing disorders. Your role is crucial in developing personalized therapy plans, implementing effective interventions, and working collaboratively with patients and their families to improve communication skills and overall quality of life.
Schedule: PRN, Weekdays, Day Shift
Primary Responsibilities and Duties:
- Conduct thorough assessments of patients’ speech, language, communication, and swallowing abilities to identify deficits and develop individualized therapy plans.
- Design and implement targeted therapy programs to address specific disorders, including speech articulation, language comprehension and expression, cognitive-communication skills, and dysphagia (swallowing difficulties).
- Educate patients and their families on therapy techniques, home exercises, and strategies to support and enhance communication and swallowing functions.
- Monitor and document patient progress, response to therapy, and any changes in condition, providing detailed reports to the supervising physician and other healthcare team members.
- Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to ensure a coordinated and holistic approach to patient care.
- Maintain accurate and up-to-date patient records and documentation in compliance with regulatory and organizational standards.
- Participate in team meetings to contribute to comprehensive care planning and decision-making.
- Stay current with advancements in speech therapy practices and participate in ongoing professional development.
- Perform all other duties assigned.
