The Hospice Certified Nursing Assistant (CNA) or Hospice Aide provides compassionate care to patients in their end-of-life journey, ensuring comfort and dignity for both patients and their families or caregivers.
Schedule: PRN, Thurs, Fri, Sat
Primary Responsibilities and Duties:
- Assist patients with personal hygiene, including bathing, grooming, and dressing.
- Monitor vital signs and report any changes to the supervising nurse.
- Administer medications and treatments as directed by the healthcare team.
- Provide companionship and emotional support to patients.
- Assist with mobility and positioning to enhance comfort and prevent bedsores while ensuring the patient's safety.
- Support families in understanding and coping with the emotional aspects of end-of-life care.
- Maintain a clean and safe environment for patients.
- Accurately document care provided and report any significant changes in the patient’s condition. This includes noting exact medication and care provided with the date and time for insurance purposes and appropriate quality of care to patients.
- Adhere to all safety protocols and infection control measures.
- Collaborate with family members and health care teams to ensure continuity of care.
- Adhere to infection control and safety procedures to protect patients and staff.
- Perform all other duties assigned
