Case Manager

Key requirements from this job

• Bachelor’s degree
• At least 1 year of geriatric experience
• OR LPN background
• Experience in settings such as:
  • Nursing homes
  • Home health
• ADvantage Case Management experience not required; training provided
• Ability to work independently
• Strong computer and typing skills
• Full-time/part-time language appears inconsistent in the posting, so I would not assume the schedule without clarification
• Case management responsibilities:
  • Initiate and monitor service plans
  • Establish achievable goals
  • Provide resource referrals
  • Meet the needs of ADvantage Program Members
• No weekends
• No holidays
• No on-call requirement
