Position Summary

The MDS RN coordinates the RAI process; audits and produces a plan of correction and manages the sign-off process for assigned areas.

How You’ll Make an Impact

• Conducts and coordinates care plan meetings as needed.
• Functions as an integral part of the Quality Improvement Program for care planning and MDS.
• Meets resident/patient and gathers information on all new admissions, re-admissions, and transfers per assigned units.
• Attends shift to shift report randomly.
• Assesses resident/patient per RAI guidelines – schedules and completes MDS following RAI guidelines.
• Follows all OBRA and PPS guidelines as it applies to the RAI process.
• Audits and Reviews resident/patient charts for documentation; discrepancies, quality, reimbursement requirements, and risk for liabilities.
• Implements plan of correction and provides education to resolve audit findings; updates interdisciplinary team and MDS Manager regarding corrections as necessary.
• Process QM audits as well as regular MDS audits for State and MDS surveys.
• Reviews support documentation for key clinical areas including wound care, falls, weight loss, dialysis, Foley catheter, and significant change.
• Keep abreast of all MDS updates regarding regulatory compliance.
• Assist with dining on units as scheduled.
• Participate in administrative rounds as assigned.
• Serves as unit champion.
• Serves as MDS manager backup.
• Collaborate with the nursing team (Nurses and CNAs) and clinical education to improve documentation.
• Review all baseline care plans for regulatory compliance.
• Utilization of PointRight as instructed.
• All other duties as assigned.

Accountabilities:

• MDS should accurately reflect the patient/resident with no allowance for deficiencies in care plans or MDS accuracy.
• Responsible for timely completion and submission of the MDS and maximizing reimbursements.
• Ensures that care plans are current and reflect the patient/resident.

What You’ll Bring

• Associate degree or equivalent from a two-year College or technical school and a minimum of 2 Years’ experience in the field.
• RAC-CT certification desirable.
• Prior experience with MDS and care plans is desirable.
